• Doctor's consultation, and injection procedure is handled by Registered Nurse.
• 1st shot: Within 30 days after purchase
  2nd shot: 2 months after the first shot
  3rd shot: 5 months after the first shot and should be completed within 6 months
• Effective in preventing 9 HPV types including 6,11,16,18,31,33,45,52,58 which may cause premalignant lesions and cancers affecting the cervix, anus, head and neck, throat mass and genital warts (Condyloma acuminata) up to 80-90%

Attention:

• The 1st injection have to completed within 30 days after purchase, and all 3 doses have to completed within 6 months
• If customer fail to complete all injection within 6 months, re:HEALTH will not provide any refund on vaccination

• After the customer completed payment from health.ESDlife, customers can call for an appointment to center 1 business days after (Call: 2156 5877 / Whatsapp: 6743 0862 - Business Hour)
• The e-report will be sent by email in 14 business days after the service.(Report will be delay when rainstorm warning or tropical cyclone signal issued by the Hong Kong Observatory)
• Starting from Sep 1, 2021, customer MUST redeem the service within 3 months once the payment is confirmed. Invalid exceeds the period.
• Injection will be done by registered nurse.
• The above items are only applicable to 1 person and can only be used once (exclude 2 person plan). They are not redeemable for cash and cannot be converted into other products and services.
• The customer must show a valid supporting document for registration before enjoying the service.
• The customer should be understood and agreed to the service arranged by re:HEALTH before using the service.
• If the package include Fasting Plasma Glucose, it must be start in 9:00 - 10:00.
• Amendment or cancellation is not allowed once the payment is confirmed, and is not transferable and refundable.
• All service(s) are not for the purpose of medical diagnosis or treatment.
• All service(s) will be provided from re:HEALTH
• In case of any dispute, health.ESDlife and re:HEALTH reserves the right of final decision.
