Terms & Conditions
I. Notes for Health Check Appointment
- After receiving the email confirming successful purchase from health.ESDlife, The Medical Center will contact the customer to schedule the time and location for the health check within 1-2 working days during office hours. Customers can also make an appointment by contacting The Medical Center at least 1 working day in advance (Contact phone Number: +86 4006089939; WeChat: 13510018256).
- Upon arrival, the staff of The Medical Center will verify the customer's name, date of birth, mobile number, and the email confirming successful purchase from health.ESDlife.
- If the customer wants to reschedule the order, please contact contact The Medical Center at least 1 working day in advance (Contact phone Number: +86 4006089939; WeChat: 13510018256).
- The validity period of the health check packages is 3 months. Customers must undergo the relevant checkup within 3 months from the date of confirmed payment. After expiration, it will be considered void.
- During the health check, if customers encounter a doctor who does not speak Cantonese, The Medical Center can arrange for medical staff to accompany and provide translation services.
- If there are any discrepancies or inconsistencies among the Traditional Chinese, Simplified Chinese, and English versions of the merchant page and the health check package pages, the Traditional Chinese version shall prevail.
II. Retrieval and Explanation of Report
- Customers can confirm the language of the report on the day of the health check (choices include Traditional Chinese, Simplified Chinese, or English).
- The health report will be completed within 7 working days after the health check. Customers can choose the following methods to view the report:
1. After the report is completed, The Medical Center will send a reminder message to the customer's reserved mobile phone number. The customer can click the link to view the report.
2. Email: Provide an email address, and The Medical Center will send the report to the customer's email upon completion.
3. Mail: Provide a mailing address, and The Medical Center will mail the report with postage collect (available for delivery to Hong Kong and Macau). - After the report is completed, customers can schedule an appointment with a doctor to explain the report. The following channels are available:
1. Telephone explanation: Need to make an appointment at least 1 working day in advance (Contact phone Number: +86 4006089939; WeChat: 13510018256). The doctor will contact the customer proactively at the scheduled time.
2. Face-to-face explanation: Need to make an appointment at least 1 working day in advance (Contact phone Number: +86 4006089939; WeChat: 13510018256). The customer should go to The Medical Center at the appointed time to listen to the doctor's face-to-face explanation. Available location for face-to-face consultation:

III. Disclaimer
- In case of controversy, health.ESDlife and The Medical Center reserve the right of final decision.
- All health check/health screening services are not for the purpose of medical diagnostic or therapeutic purposes. When there is any sign of symptom/disease in your health, please consult Doctor immediately for diagnosis and treatment.
- The Merchant is the service provider of this Service/Product. ESD Services Limited (“health.ESDlife”) is not the service provider of this Service/Product. Health.ESDlife is irresponsible to any loss, injury or law action caused by using this service/product. Any claims and inquiries should be addressed to the respective Merchant.
