I. Checkup Appointment Guidelines

• After the customer receives the confirmation email of successful payment from health.ESDlife, Yunshan Medical will contact the customer to schedule the time and location of the checkup within 1-3 working days during office hours. Customers can also contact Yunshan Medical at least 3 days in advance (Contact number: +852 5124 1393).
• Upon arrival at the venue, the staff of Yunshan Medical will verify the customer's name, date of birth, mobile number, and the successful purchase email from health.ESDlife.
• The checkup plan is valid for 3 months, and the customer must undergo the relevant checks within 3 months from the date of confirmed payment, otherwise it will be void.
• During thecheckup, if the doctor does not speak Cantonese, Yunshan Medical can arrange for medical staff to accompany and provide translation services.

II. Checkup Report Collection and Explanation

• Electronic Version: Yunshan Medical will notify customers via SMS to log in to the "Yunshan Medical" WeChat public account to view the report (The SMS will include the electronic inquiry account and password). If customers prefer to receive notifications via email / WeChat, they must provide their email address / WeChat.
• Paper Version: Customers must bring the checkup report collection form or SMS notification and come to checkup center to collect the report.

• Phone Explanation: Need to make an appointment at least 3 days in advance for a specific time (Appointment contact number: +852 5124 1393), and the doctor will contact the customer at the scheduled time.
• Face-to-Face Explanation: Need to make an appointment at least 3 days in advance for a specific time (Appointment contact number: +852 5124 1393), and the customer should go to the health check center at the agreed time to listen to the doctor's explanation.
