* If it is considered as inapproriate to get vaccinated, HKD1700 will be charged for Dr Consultation. The remaining amount shall be returned. health.ESDlife and Adventist Medical Centre have sole discretion to make the final decision.

• Package Include: Pre and Post Procedure consultation, Designated endoscopist procedure fee, Endoscopy room fee, Medication for intravenous sedation (IVS), Biopsy (tissue sampling), Photographic documentation x 1 set, Procedure Report， CLO test report
• Package exclude: Laboratory charges, Subsequent treatment based on pathology report, fee of polyp removal and related consumables, Additional medication and materials, Emergency Services beyond the routine procedure, additional photos/ images, oral medications after examination
• The dosage and brand of the laxative will be determined by the doctor after on-site evaluation. Therefore, its cost is not included in the treatment package price. Customers are required to make an additional payment for it at the clinic.

*The final fee for the endoscopy procedure will be determined based on the actual findings during the examination. If polyp removal or biopsy (tissue sampling) is required during the procedure, the relevant charges will be adjusted accordingly due to the additional medical time involved and the number of polyps handled. The client must pay any outstanding balance in accordance with the final settlement issued by our centre upon completion of the examination.

• Histopathology: $1,000 each bottle

Term and Condition

1. All particip4ants should be referred by an Adventist Medical Center specialist.
2. The above packages are not suitable for tose with servere chronic diseases, such as high blood pressure, heart disease or sleep apnea, etc.
3. The above packages are only valid for day case in Adventist Medical Center appointed date and time.
4. The above packages only include procedures that involve examination or diagnosis. If treatment is required such as hermorrhoid ligation, additional fee will apply. Please contact Adventist Medical staff for details.
5. Extra charges may be incurred if complications arise during the procedure, requiring additional treatment or use of medical that are not included in the package. Please contact Adventist Medical Center staff for details.
6. Adventist Medical Center and health.ESDlife reserves the rigtht to make all final decisions.

Adventist Medical Center Terms and Conditions on Health Check Services:

• After successful payment, the Customer receives confirmation email from health.ESDlife, Adventist Medical Center (Taikoo Place) will call the Customer to schedule the appointment within 1-2 working days. Customer can call Adventist Medical Center (Taikoo Place) for any enquiry. (tel: 2309 5000)
• Customers must present their identity cards and print the order confirmation letter on the appointment day.
• The health check package is valid for six months ( from the date of purchase ) or it will be forfeited.
• Under normal circumstances, all medical reports will be ready around 7-10 working days (excluding Saturday, Sunday and public holiday). A delay in arranging explanation session may due to the following reasons: 1. indicate the specific selective date 2. Some tests take more time.
• Amendment or cancellation on package/plan is not allowed once the payment is confirmed.
• All tests are not for the purpose of medical diagnosis or treatment.
