Package includes: Doctor and Anesthetist fees, Endoscopy room fees, Polyp removal (up to three small polyps in the stomach and intestines respectively. The number of Polyps in the stomach and intestine is not interchangeable), Medications used in the endoscopy room, Detailed Reporting: Receive a comprehensive report with photographs documenting your procedure to keep track of your health status, The doctor completes one copy of the insurance claim form.

1. This package includes fees for a specialist doctor, anesthetist, removal and testing of three small polyps from the stomach and intestines respectively, as well as the doctor's consultation fees for both the first and follow-up visit (report explanation). However, it does not include hospitalization and discharge medications.
2. Due to different procedures, there is an additional charge for the removal of large polyps. For details, please inquire with our doctor.
3. This plan must be assessed by a doctor to determine its suitability. If, after assessment, the doctor finds that the patient is not suitable for the endoscopy, a one-time consultation fee will need to be paid on the day of the appointment (Monday to Saturday $285; Sunday, public holidays, black rainstorm warnings, and typhoons of signal No. 8 or above $395). The package amount paid during online purchase will be fully refunded by heath.ESDlife.

Terms & Conditions:

• The above endoscopy package is performed by specialists in our hospital.
• The package includes a six-hour hospitalization period. An additional fee of $210 per hour will be charged for any extra hospitalization time.
• The above package fee includes the consultation fees for both the initial visit and the follow-up.
• The above package does not include hospitalization and discharge medications.
• The doctor will have full discretion to determine whether the package fee applies to individual patients based on their condition. If the doctor deems the relevant endoscopic procedure unsuitable for a patient, a one-time consultation fee will be charged, and the amount paid for the package will be fully refunded by health.ESDlife.
• The package is only applicable to individuals aged 16 to 75 years old, with a Body Mass Index (BMI) of 29 or lower, and who are not taking anticoagulants or aspirin.
• Individuals under 18 must be accompanied by a parent or guardian.
• Additional treatments will incur extra charges, such as for foreign body removal, hemostasis, hemorrhoidal ligation, tissue testing, or the use of additional instruments.
• The package includes the removal of three small gastric polyp and three small intestinal polyp from the patient (not interchangeable). The removal and testing of more than three samples or larger polyps will incur additional charges. Please consult the doctor for details.
• By purchasing this endoscopy package, the patient agrees to allow the doctor, based on professional judgment, to remove any relevant polyps or samples during the monitored anesthesia period and is willing to pay any additional costs incurred.
• The above package includes the doctor completing the insurance claim form once. If an additional form is required, the hospital will charge an additional administrative fee of $340. The fee may be adjusted due to different circumstances.
• The package discounts cannot be combined with other offers.

Payment and Confirmation of Purchased Health Check Items
After the customer receives the confirmation email of successful payment sent by health.ESDlife, a related email will also be sent to the designated email address of the Evangel Hospital (The Hospital). The physical checkup or medical services purchased by the customer have been successfully booked. Unless Evangel Hospital has additional information to notify the customer, the customer only needs to visit the hospital on the specified date and time for the scheduled check-up or service.

Unless due to severe weather conditions, once the service is paid for and booked, the hospital will not accept changes to the date.

Age
The health check program or medical services are only applicable to individuals aged 18 and above. (Except for some special plans)

Validity

The purchased service is valid for 3 months starting from the date of payment. Clients must undergo the relevant procedure within the validity period and must make an appointment at least one month before the expiration of the validity period. Any appointments made after this period will be void, and no refunds will be issued.

Collection of Checkup Reports
The Hospital's physical checkup service includes the doctor's assessment fees before and after the service. Generally, the Hospital will schedule an appointment for the customer to return to collect the report around 14 working days (excluding Saturdays, Sundays, and public holidays).

Note The waiting time for report explanation may vary depending on different circumstances (such as the time required for specific test items or if the customer specifies a particular time). If the doctor finds any abnormalities in the test reports, the customer will be notified as soon as possible for immediate follow-up.

Collection Method
Customers must visit the hospital in person to collect the report, which our doctor will explain the report in details.

Terms and Conditions
1. Customers must collect their medical examination report within eight weeks after the check-up; otherwise, they will be required to pay an additional doctor’s consultation fee (Mon - Sat: $285; Sun & Public Holidays: $395).
2. Customers should arrive at the hospital at least 10 minutes before the scheduled appointment time. If customers arrive over an hour late, miss the appointment, or do not attend, the hospital will not provide rescheduling or refund arrangements.
3. If customers require additional copies of the report, the hospital will charge an administrative fee.
