Evangel Hospital Specially Selected Comprehensive Adult Health Checkup
Suitable for men and women aged 18 and above. Includes several important health indicators: Ultrasound for liver, gallbladder and spleen; heart, lung, liver, kidney, and thyroid function tests; detailed eye examination; blood tests, lipid tests, urine tests and cancer markers . Evaluations and follow-ups are conducted by a general practitioner or Family Medicine specialist doctor both before and after the checkup.
Payment and Confirmation of Purchased Health Check Items
After the customer receives the confirmation email of successful payment sent by health.ESDlife, a related email will also be sent to the designated email address of the Evangel Hospital. The physical checkup or medical services purchased by the customer have been successfully booked. Unless the hospital has additional information to notify the customer, the customer only needs to visit the hospital on the specified date and time for the scheduled check-up or service.
Unless due to severe weather conditions, once the service is paid for and booked, the hospital will not accept changes to the date.
Age
The health check program is only applicable to individuals aged 18 and above.
Validity
This Health Checkup Plan is valid for 3 months starting from the date of payment. Clients must undergo the relevant checkup plan within the validity period and must make an appointment at least one month before the expiration of the validity period. Any appointments made after this period will be void, and no refunds will be issued.
Collection of Checkup Reports
The hospital's physical checkup service includes the doctor's consultation fees before and after the check-up. Generally, the hospital will schedule an appointment for the customer to return to collect the checkup report around 14 working days (excluding Saturdays, Sundays, and public holidays).
Note The waiting time for report explanation may vary depending on different circumstances (such as the time required for specific test items or if the customer specifies a particular time). If the doctor finds any abnormalities in the test reports, the customer will be notified as soon as possible for immediate follow-up.
Collection Method
Customers must visit the hospital in person to collect the checkup report, which our doctor will explain the report in details.
Terms and Conditions
1. Customers must collect their medical examination report within eight weeks after the check-up; otherwise, they will be required to pay an additional doctor’s consultation fee (Mon - Fri: $285; Sat, Sun&Public Holidays: $395).
2. Customers should arrive at the hospital at least 10 minutes before the scheduled appointment time for the health check. If customers arrive over an hour late, miss the appointment, or do not attend, the hospital will not provide rescheduling or refund arrangements.
3. If customers require additional copies of the report, the hospital will charge an administrative fee.
