Health Check Precautions

1. Please maintain a light diet the day before the check-up, avoid vigorous exercise, and ensure adequate sleep.After 8 p.m., fasting is required. On the day of the check-up, you must fast in the morning (no food, water, or medication), except for those with high blood pressure, who may take their medication before the check-up (only drink enough water to swallow the pills). Customers with diabetes, asthma, heart disease, etc., should bring their medication.
2. If you have any special medical conditions, please inform the attending physician truthfully (such as pregnancy, cardiovascular and cerebrovascular diseases, diabetes, etc.).
3. Please wear loose and comfortable clothing. For X-ray examinations, it is recommended to wear cotton underwear and avoid clothing with metal sequins or metal buttons (which need to be removed on-site), as well as metal items such as necklaces, mobile phones, keys, and coins (which need to be taken off on-site). For ultrasound and ECG examinations, ladies should avoid wearing dresses or tight-sleeved shirts.
4. When collecting stool samples, if there is mucus or blood, please take more of that part. For urine samples, it is best to collect the midstream urine from the first morning urine.
5. For gynecological ultrasound examinations, please keep the bladder full (the bladder is full when you feel the urge to urinate but are not overly bloated).
6. During the check-up, please ensure that you undergo each item of the check-up as scheduled to avoid missing any tests, which could affect the assessment of your health condition.
7. If the examinee voluntarily skips certain tests due to personal reasons, please sign in the corresponding item column to confirm.
8. After completing the check-up, please hand in the health check process guide form to the front desk staff to confirm whether there are any missed items.

• After receiving the email confirming successful purchase from health.ESDlife, C+ Health CKJ (Shenzhen) Hospital will contact the customer to schedule the time and location for the health check within 1-2 working days during office hours. Customers can also make an appointment by contacting C+ Health CKJ (Shenzhen) Hospital at least 1 day in advance (contact phone number : +852 3848 1047).
• Upon arrival, the staff of C+ Health CKJ (Shenzhen) Hospital will verify the customer's name, date of birth, mobile number, and the email confirming successful purchase from health.ESDlife.
• If the customer wants to reschedule the order, please contact C+ Health CKJ (Shenzhen) Hospital at least 1 day in advance (contact phone number and WhatsApp: +852 3848 1047).
• The validity period of the health check packages is 3 months.Customers must undergo the relevant checkup within 3 months from the date of confirmed payment. After expiration, it will be considered void.
• During the health check, if customers encounter a doctor who does not speak Cantonese, C+ Health CKJ (Shenzhen) Hospital can arrange for medical staff to accompany and provide translation services.
• If there are any discrepancies or inconsistencies among the Traditional Chinese, Simplified Chinese, and English versions of the merchant page and the health check package pages, the Traditional Chinese version shall prevail.

II. Retrieval and Explanation of Report

• The health check report is provided in Simplified Chinese.
• The health check report will be completed within 14 days. Customers can choose the following methods to view the health check report:
  1. After the health check report is completed, C+ Health CKJ (Shenzhen) Hospital will send a reminder message to the mobile phone number provided by the customer. Click the link to view the electronic report.
  2. Customers can provide an email address, C+ Health CKJ (Shenzhen) Hospital will send the health check report to that email address for viewing.
• After the health check report is completed, customers can schedule a doctor to explain the report through the following channels:
  
  1. Telephone explanation: Customers need to make an appointment at least 1 days in advance for a specific time (phone number: +852 3848 1047), the doctor will proactively contact the customer at the scheduled time.
  2. Face-to-face explanation: Customers need to make an appointment at least 1 days in advance for a specific time (phone number: +852 3848 1047). The customer should go to C+ Health CKJ (Shenzhen) Hospital at the appointed time to listen to the doctor's face-to-face explanation. Available locations for face-to-face explanation:
     1. Part of 2/F, 3-7/F, Zone G, 2nd Floor, West Railway Station, Heping Road, Nanhu Street, Luohu District, Shenzhen, China

III. Disclaimer

In case of controversy, health.ESDlife and C+ Health CKJ (Shenzhen) Hospital reserve the right of final decision.
