• After the customer completed payment from health.ESDlife , HKPV will call the customer to make an appointment within 1 to 2 business days.
  Customers can also call for enquiries to center 1 day after the order is confirmed (Hotline: 2156 5865 / Whatsapp: 9135 5701)
• This transaction is subject to the assessment by nurse for the suitability of vaccine injection. If patient is not suitable for the vaccine injection after consultation, the cost will be refunded. Under any condition, doctor consultation fee $400 will be charged and nonrefundable after the consultation by request.
• The service(s) can use at hkpv centre only.
• The service(s) are for one new customer only.
• Identity verification documents are required for registration before service(s).
• For those who are under 18 and would like to perform our service(s) , they must be accompanied by their parent(s) and sign the vaccination consent form).
• Service(s) are valid for 3 months from the date of purchase. After which, the confirmation letter will be deemed invalid. (Customers who purchased from March 1, 2020 will have their validity extended to 6 months.)
• No refund is allowed once the Order has been confirmed.
• The offer cannot be redeemed for cash.
• All Health Check Services and/or Vaccination Services are not intended for medical diagnostic or therapeutic purposes.
• Before service(s), customer should understand and agree with the service(s) details.
• Should there be any questions, please feel free to contact our consultants.
• In case of any disputes, the decision of Hong Kong Professional Vaccine Limited shall be final.
